Behavioral Services

In 2013, Roots added on-site counseling with a mental health professional for individuals with mental health diagnoses, including co-occurring substance use disorders. Roots behavioral health programs and services provide mental health services to all populations, including but not limited to: pediatrics, transitional age youth, adolescents, adults, and seniors, and the re-entry population.

Individual & Group Visits

Roots meets the community where it is. Our services span beyond our locations and into homeless encampments, shelters and homes. We provide quality care and wraparound programs to empower our patients on their journey to wellness.

Collaborative Care

Our clinicians work collaboratively with patients, fusing primary and preventive care with behavioral and mental health services to assist our patients in their quest toward a sound body and mind.
