Roots helps formerly incarcerated individuals of our community get reacclimated into society with navigation services that assist them in finding employment, housing, and much more.

Clients can receive navigation services through a variety of programs across our many sites.

Healthy Measures Warm Handoff Initiative

 

The Healthy Measures Warm Handoff Initiative helps formerly incarcerated individuals achieve successful, healthy reintegration into society. Healthy Measures Warm Handoff participants receive support and coaching from Roots Health Navigators who have also been impacted by incarceration. 

Health Navigators empower participants through:

  • Individualized plans for achieving “whole health”
  • Benefits enrollment
  • Health education and navigation
  • Assistance with removing barriers to employment, housing and other critical needs

Safe Landing at Santa Rita Jail

 

The Safe Landing Pilot Program launched in Summer 2020 in the parking lot of Santa Rita Jail to provide transitional services to individuals as they are leaving the county jail. In 2021, the Safe Landing Shuttle service was added to provide safe transport for individuals to Dublin BART and Oakland from the jail.

Services include:

  • Supplies distribution
  • Benefits enrollment
  • Coordinated Entry Housing Assessments
  • Linkage to physical and behavioral healthcare, care management and housing.
