Community Engagement & Transition Services

Roots and our numerous collaborative partners work to provide resources and direct services at the time and place of need. Our community events bring engagement as well as timely and trustworthy information related to health and wellbeing.

Systemic injustice burdens our communities with lack of access to resources and information. Roots teams up with other community based organizations to combat these inequities and serve marginalized communities.

In addition to medical and supportive services provided by Oakland Street Team Outreach Medical Program (STOMP), teams of outreach workers engage in street and encampment outreach every day. Services include supplies distribution, benefits enrollment, and coordinated entry housing assessments.

Roots helps formerly incarcerated individuals of our community get reacclimated into society with navigation services, barrier removal, employment and training, and housing assistance.
