Roots believes in taking services to the community with the goal of engaging more people – especially those who need it most.

We provide:

Mobile Healthcare through OaklandSTOMP (Street Team Outreach Medical Program)

Mobile Enrollment into Medi-Cal, CalFRESH and our clinical services through our HealthyCONNECTIONS program

Residence-based care for those unable to come to our clinics, and

Community Based Services through our community partners
