Roots Community Health Center is seeking an experienced Program Coordinator. The Program Coordinator for East Oakland Ready for School at Roots Community Health Center will have a primary role in the oversight, coordination, and implementation of a place-based strategy that will assess and evaluate resources, strengths and risks for families with children 0-5 who are preparing for healthy development to successfully enter kindergarten. He/she will be an integral part of the team implementing strategies in the Eastmont, Elmhurst, Stonehurst, Sobrante Park and Brookfield neighborhoods, and the East Oakland Ready For School Initiative.
Roots Community Health Center is seeking to fill several Health Navigator positions over the next three months. Navigators are the largest job class at Roots, and they are an integral part of our Whole Health model. Navigators conduct in-depth initial assessments, benefits enrollment, engage in face-to-face visits and make frequent outreach calls to their clients based on their needs. Navigators serve as the primary coach for individuals and/or families, facilitating the initial introduction to Roots programs and services, maintaining ongoing contact with their panel of individuals and/or families. They also assist with access to: physical health services, mental health services, substance use services, family/support system, counseling needs, housing, food, clothing, hygiene items, healthy communication, relationship coaching, etc.
These are the current and upcoming Navigator positions for which we are/will be hiring:
– Oral Health Navigator: 75-100% time, open now
– Complex Care Navigator: 100% time, open: September
– School-based Navigator: 50-100%, open: August
– Family Navigators (2): 100% time, open: October

Roots Community Health Center is seeking a dedicated Development Specialist. The Development Specialist must be committed to the mission, vision, and services of Roots and able to communicate these in written and verbal format to potential supporters, stakeholders, funders and collaborative partners. This position ensures consistency and accuracy of messaging that will broaden the organization’s reach. The Development Specialist supports current programs as well as strategic growth and development through data gathering and analysis; internal and external communications; and event coordination with the goal of broadening and deepening the impact of Roots’ work.
